Lisinopril was the least effective in blood pressure control and is associated with a high risk of death. However, it was effective in reducing albumin excretion in the urine in patients with diabetes. The U.S. Food and Drug Administration approved captopril for diabetic nephropathy.

Thirteen studies (1022 patients) with 4 drugs (captopril, enalapril, lisinopril, and ramipril) and placebo were included. Lisinopril was associated with higher all-cause mortality compared with placebo (OR 65.9, 95%CrI 1.91 to 239.6) or ramipril (14.65, 1.23 to 49.5).

Both captopril and lisinopril are effective ACE inhibitors for the treatment of CHF, with each having its own advantages. Lisinopril generally shows superior efficacy in improving exercise duration, LVEF, and blood pressure control, particularly in patients with renal impairment and the elderly.
